You might be able to assume from this sent that John ended up in
California as a young fellow (Calif. Normal School ) and then came back to PA and attended college (graduate medical department University of Pittsburgh); however, since there is no census for 1890, it makes it tough

I don't' find anything definitive yet in 1900, but here he is in 1910:

In 1900, in Allegheny, I find a John, age 22, b. Oct 1877, who is a student
(upon enlarging, next word is medicine). There is no father in the household; mother is Margaret; this may be the Maggie above in 1880 this John has a sister, Hattie, but her age, birth year are unreadable (faint, might be Mar 1874/84); there is an Andrew, nephew, b. Dec 186?, age 3?

California State Normal School (now, San Jose State University <http://en.wikipedia.org/wiki/San_Jose_State_University>) established by the California Legislature on May 2, 1862. A second campus was created in Los Angeles in 1882. In 1887, the California legislature inexplicably dropped the word "California" from the name of the San Jose and Los Angeles schools, renaming them "State Normal Schools". Later Chico (1887), San Diego (1897), and the others became part of the State Normal School system. In 1919, the State Normal School in Los Angeles became the Southern Branch of the University of California (now the University of California, Los Angeles <http://en.wikipedia.org/wiki/University_of_California,_Los_Angeles>). In 1921, the State Normal Schools became the State Teachers Colleges, but the loss of the word "California" had taken its toll and by this time most of the campuses became identified by their city name plus the word "state" (e.g, "San Jose State", "San Diego State", "San Francisco State").
